That I am participating in The Clanfield Centre’s other activities using The Clanfield Centre’s premises, equipment and facilities. I recognize that these classes require physical exertion that may be strenuous and in rare cases cause physical injury. I am fully aware of the risks and hazards involved.
Health and safety are of paramount importance to The Clanfield Centre and risk assessments have been undertaken to minimize risk. Users will undertake all activities at their own risk and no responsibility will be taken or accepted for accidents or injuries whilst undertaking any centre activity.
I understand that it is my responsibility to consult a physician prior to participating in any activities at The Clanfield centre. I represent and warrant that I am physically fit and that I have no medical condition that would hinder my partaking in the classes involved.
I have considered and understand that I am in control of my own body and actions and will endeavor to ensure I go at my own pace to ensure minimal risk of injury or accident whilst partaking in any activity within The Clanfield Centre.
In consideration of being permitted to participate in sporting activities, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which might incur as a result of participating in the classes and activities.
In further consideration of being permitted to participate in activities, I knowingly, voluntarily and expressly waive any claim I may have against The Clanfield Centre, their instructors and staff for injury or damages that I may sustain as a result of participating in the classes.
I understand that all sessions must be booked and paid for in advance and The Clanfield centre cannot be responsible for my individual bookings. I understand that the Clanfield Centre cannot be held responsible for any disappointment regarding bookings.
I agree to adhere to the 48-hour cancellation policy for every class or booking. I understand that If I cancel a session with less than 48 hours’ notice, full charge of the class or booking will be taken. Within this 48-hour cancellation I understand that sessions cannot be transferred or refunded. Your booking cannot be reserved without payment.
We expect our customers to behave in a considerate manner and will not accept any inappropriate behaviour towards other members or our colleagues. We reserve the right to refuse admission for members behaving in such a manner.
I understand that The Clanfield Centre will not accept any liability for loss, damage or theft of any personal items and these items are left at their own risk.
I understand that my booking time includes set up of equipment and possible take down time of equipment before the next booking.
I understand that court hire is for social use only, and the court cannot be sublet by charging other users to take part in any activity unless specifically booked as a vendor with an up to date hirer agreement, signed by the Centre Manager.
I have read the above release and waiver of any liability and fully understand its contents. I voluntarily agree to the terms and conditions above.
